What causes a rectifier to burn out

Judging by your described sequence, I'd say the rectifier (5V4) went bad, you plugged a new rectifier (5AR4), which bumped the bias up a fair amount, resulting in taking out a power tube. Hard to say which tube actually precipitated the original failure.
